The medical term for overdeveloped breast in men is gynecomastia. One-third of male teenagers commonly experience breast enlargement, 80% of that number will return to normal in three years or less. Highly motivated male athletes may be frustrated with the look of their body after reaching a fitness plateau and may resort to the use of anabolic steroids. Steroids may increase muscle mass but at the expense of potential breast enlargement as well as aggressive behavior or changes in personality. The best candidates are men who are in good physical and psychological health.

A surgeon at The Aesthetic Center for Plastic Surgery will perform an examination to determine the best treatment for you. If surgery is recommended, in most cases the surgeon can remove the tissue through small incisions on or around the breast. The breast is approximately 50% fat and under the direction of a skilled plastic surgeon, small specialized cannulas can carefully sculpt the ideal male breast by removing the excess fat. With new liposuction techniques, the correction can be completed in about one to 2 hours under local anesthesia.

Surgery may be performed as an outpatient. After your procedure the physicians at The Aesthetic Center for Plastic Surgery may offer a visit to your home by a plastic surgery nurse, trained to assist in your recovery. You can expect to return to work in three to 4 days. Your surgeon will probably tell you to avoid heavy exercise for two to three weeks.

Hair Replacement

Hair loss is primarily a combination of hormones, aging, and family history. Theories about vitamin deficiencies, poor circulation, or excessive hat wearing have been largely disproved. Shampoos and solutions applied directly to the scalp have not provided dramatic results. Hair replacement has proven to be a successful treatment for baldness, especially in men with male pattern baldness. The replacement procedure uses existing hair from the back and sides of the head, known as donor areas. Mini-grafts and micro grafts are usually the treatment of choice for filling in thinning areas.

Good candidates should have dense hair growth at the back and sides of the head. A small strip of hair is removed usually in the back of the head and then each hair follicle is dissected under magnification. Single, double and triple hair grafts are placed several millimeters from each other in the balding area. Mega session, utilizing more than 1000 grafts may be necessary. Your physician may choose to fill in between the hair plugs at a later time with additional grafts. He will take great care in the harvesting and placing of the grafts to create the most natural look possible.

Some procedures are done under local anesthesia and some under general. The type of anesthesia and the operating room time varies with the procedure of choice. More dramatic results to treat baldness can be achieved with surgically reducing the balding area in the procedure called a scalp reduction. Factors for any procedure, such as hair texture, color, and curliness, may affect the final result. Most patients can return to work after this procedure in a few days, resuming activities in about a month.

Abdominal Etching (The Athletic Look)

Abdominal etching, more commonly referred to as the rippled or washboard look, is a liposuction procedure designed to give patients a new abdomen and waist contour. Men find this procedure especially helpful in defining the subtle abdominal muscles producing the six-pack muscle ripples so popular today. We are proud to say that physicians at The Aesthetic Center for Plastic Surgery originated and developed this specialized technique and have led the nation in performing these cases receiving national and international recognition for their efforts.

The procedure involves liposculpture which creates subtle ripples in the abdominal area allowing the underlying abdominal muscles to show greater detail. Fat is selectively removed through a small hollow tube inserted through one or more tiny incisions. Most incisions are hidden in the natural body creases of the body leaving very small scars.

For males a typical 36-40 inch waist can be safely reduced to 28-32 inches. For those patients who exercise on a regular basis or with excellent skin tone, a better defined washboard stomach can be obtained in just a few hours. This is not a substitute for weight loss and the best candidates are of relatively normal weight with isolated pockets of fat.

Waist Contouring

Just as women have the hormonally-induced saddlebag thighs, so men have the hormonally-induced spare tire around their middle section. This area is made up of dense, fibrous tissue which in the past was more difficult to remove. The doctors at The Aesthetic Center for Plastic Surgery have pioneered new liposuction techniques to achieve a more athletic appearance for the male patient. Liposuction is the removal of unwanted fat from the body.

A small incision is made in the skin just large enough to accommodate a small hollow tube called a cannula. The cannula suctions the fat from selected areas of the body to create the desired contour. Any person who can pinch 3/4 inch of fat in the abdomen can benefit from this type of liposuction. Diet and exercise can only reduce the spare tire so much. The board certified surgeons at ACPS have over ten customized treatments designed to enhance the muscular physique of a bodybuilder or the former athlete now 20-50 lbs heavier.

Significant body fat reduction can result in a longer life, reduced lower back strain and allow a more intense exercise program to diminish muscular skeletal pain. These procedures are performed on an outpatient basis, usually under general anesthesia. The patient is able to be back at work in three to five days and resume sports activities in 3 weeks.

Cellulite

Cellulite is a hormonally-induced condition, typically found in the lower body of women. Cellulite is fat deposits in the lower extremity where skin tends to accumulate in engorged fat compartments. Over time, fat lobes under the skin increase in volume and the vertical connective fibers between the lobes shrink, causing the fat lobes to press out against the skin. This produces the rippling or orange peel appearance of the skin. The physicians at The Aesthetic Center for Plastic Surgery have been evaluating new non-surgical methods for reduction of cellulite, and are pleased to announce that they are now offering these services.

The new technology works by using a special instrument to gently massage and lift specific areas of the body. The connective tissue is stretched and rejuvenated, resulting in better circulation, skin retraction and skin tone. Actual loss of inches is seen in some patients. Patients having 1 to 2 treatments a week will begin to see results in about 4 to 6 weeks. Each treatment lasts about thirty minutes.

After you have achieved a desired result, a maintenance program can be developed to assist you in maintaining your look. Like the therapeutic massage offered by ACPS, these treatments will leave you feeling rejuvenated and refreshed. This method can be used alone, or before and after liposuction to enhance surgical results.

Ultrasonic Technique

A new method, called Ultrasonic Assisted Liposuction, or UAL, is now being used in conjunction with tumescent liposuction techniques. UAL works by converting ultrasonic energy into heat, literally melting the fat cells, allowing the remains to be suctioned away. This method reduces the amount of trauma to the tissue, since less force is needed by the surgeon, leaving fibrous tissue and vascular structures intact. There is minimal blood loss, less bruising, and better retraction of tissue.

This technique is promising, but there is a danger of burning the skin. The physicians at the Aesthetic Center for Plastic Surgery recommend a combination of UAL and traditional liposuction techniques to achieve the desired results. A consultation with one of our board certified surgeons will determine the techniques best for you. And remember, it is advisable that you choose a board certified plastic surgeon who is affiliated with accredited hospitals and surgery centers, to perform your liposuction procedure.

Tumescent Technique

The tumescent technique for liposuction is still the most commonly used method for achieving desired results in contouring areas such as the waist, hips, thighs and buttocks. This technique works by injecting controlled amounts of a special fluid under the skin. This fluid causes the fat to swell such that it is easier to remove.This fluid also numbs and constricts blood flow to the area to reduce blood loss. Because there is so little blood loss, there is minimal bruising.

Medication is also added to this fluid such that post-operative pain is reduced for 18 to 24 hours after surgery. After the fluid is injected, a special hollow tube called a cannula is connected to a suction machine and then inserted under the skin. The cannula is moved back and forth to create thousands of tiny tunnels where the fat has been removed. After surgery, a compression garment, or girdle, is put on the patient. The FDA recently approved BOTOX® as an effective way to treat wrinkles on the forehead, between the brows, crow's feet (eye area) and around the mouth. Patients say that they have a softer, friendlier, more rested appearance after BOTOX®. BOTOX® has also been used to treat excessive sweating (hyperhidrosis) in the palms and armpits.

A mini neck lift is effective with injections of BOTOX® in neck bands. The effects of BOTOX® are temporary, usually returning within four to six months. Lines around the mouth return in 2-3 months. BOTOX® is injected directly into the muscle through a small needle in the physician's office. Patients can return to work that day and in most cases could participate in a social engagement that evening.

BOTOX® (Allergan Irvine, CA) is purified Botulinum Toxin Type A produced by the bacteria Clostridium botulinum. BOTOX® works by binding with motor nerve terminals in the body to inhibit the release of a chemical called Acetylcholine. When injected locally into a muscle, BOTOX® produces a reversible paralysis of that muscle. The FDA also has approved Botox® for use in treating conditions known as strabismus and blepharospasm or disorders of the facial nerve in patients aged 12 and above.

There are risks associated with BOTOX® use; however, complications are usually temporary because of the non-permanent nature of BOTOX®. As we age, our skin loses tone and begins to sag. However, skin frequently has tremendous ability to retract into a more youthful position. Even some relatively young patients prematurely age in the neck because of a laxity of the neck muscle and accumulation of fat in this area. Liposuction can often relieve the neck area of unwanted fat, but your doctor may combine liposuction with tightening of the neck muscle as well to give a longer lasting result.

This outpatient procedure is called platysmaplasty and may be performed with either "twilight" or general anesthesia. Collagen Replacement

Collagen is a substance that occurs naturally in the second layer of skin. It acts as a support structure for the skin, and provides a framework for the growth of cells and blood vessels. As a person ages, the constant stress of smiling and squinting causes the collagen to break down. Creams that have collagen in them only serve as moisturizers and can do nothing to replace lost collagen under the skin. In most cases, collagen lost to wrinkles and lines can be replaced with a series of injections under the skin using Purified Bovine Collagen Bovine collagen can be purchased by plastic surgeons from an FDA approved source.

There are new techniques being developed that allow the harvesting of a patients own collagen for re-injection. One of the most obvious signs of aging occurs around the mouth. Lines develop around the mouth, the lip tissue breaks down and the lips droop and retreat into two thin lines. The lips lengthen as a result of gravity. An inversion of the vermilion, or the pink portion of the lip is due to a gradual shrinkage of our jaw bone and teeth. This contributes to a gradual distortion of the lip. The corners of the mouth turn down, and the lower lip loses its youthful appeal.

Lip enlargement is accomplished in a multitude of ways. Some are more permanent than others. Lip improvement with the CO2 laser can remove fine lines and wrinkles around the mouth. Dermabrasion, chemical peels, collagen injections, and micro-fat injections can create significant improvements. In some cases more than one procedure may be needed to achieve the desired appearance. These procedures are usually performed on an outpatient basis. Recovery time depends upon the procedure performed.
